Registration Requirements

Expanded Transitional Kindergarten

Nevada Avenue Elementary has the new Early Childhood Program for children who turn 5 between December 2, 2015 and June 2016.  The program begins on August 18, 2015.  Children will learn language, literacy and math skills to prepare them for kindergarten and beyond.  Priority is given to families based on residency, meal application data, and birth date.  This program is a two year program.  The first year is preparing students for kindergarten while the second year is kindergarten.  If you are interested, please stop by the School Office.  

 

Transitional Kindergarten

The Transitional Kindergarten Program for children who turn 5 between September 2, 2015 and December 1, 2015.  The program begins on August 18, 2015.  Children will learn language, literacy and math skills to prepare them for kindergarten and beyond.  Priority is given to families based on residency, meal application data, and birthdate.  This program is a two year program.  The first year is preparing students for kindergarten while the second year is kindergarten.If you are interested, please stop by the School Office.  

 

Kindergarten

The new STATE IMPOSED KINDERGARTEN AGE REQUIRMENT  - Children must turn 5 years old on or before September 1, 2015.  September 1st is now the permanent cut-off for kindergarten.

Registration Documentation

Applications for registration in grades Preschool through Fifth Grade are available in the Main Office between the hours of 8:00 a.m. and 3:45 p.m.  New families who reside within our attendance area may enroll at any time during the school year.  Applications must be turned into the school office in person.  Applications will be processed and accepted once all of the requirements listed below are submitted.  A printable list of enrollment requirements is available here

The following documents are needed to register:

  • Completed Enrollment form.
  • Completed Health History - to be completed by parent/guardian.
  • Legal proof of birth.  Must bring an original of one of the following - birth certificate, hospital birth certificate, passport or baptismal certificate
  • Picture identification of the parent/guardian who is enrolling the child.
  • Verification of address.  All documents must be current and must identify the parent or legal guardian.  They must show the current date, as well as residential address.  Original documents are required.  Acceptable forms of address verification include - Utility Bills (DWP or Gas), property taxes, rental contract, official government mail (CalWorks, Social Security), property deed
    • ****Phone bills, credit card bills, and Identification are not acceptable forms of address verification.****
  • Record of immunizations must be current for the flowing and indicated the month/date/year/administered:
    • Polio - 4 doses
    • DPT - 5 doses
    • Hepatitus B - 3 doses
    • MMR - 2 doses
    • Varicella (chicken pox) - 1 dose
  • If your child is enrolling a public school for the first time, you must turn in the following items as well.
    • Examination by Physician card (included in packet).  This is required for first grade entry.  Examinations may be received with kindergarten enrollment.
    • Oral Health Assessment completed by Dentist (included in the packet).  This is required for Kindergarten and First grade enrollments.
